Patient Information

Please arrive at least 15 minutes before your scheduled appointment time.

Please bring the following items with you:

Referral from your Pediatrician/Primary Care Physician.
Name, address, phone, and fax number of your Primary Care/Referring Physician.
Name, address, phone, and fax number of your Pharmacy.
Copies of lab work, x-rays, MRI scans, etc. (If you do not have copies of the lab work/scans, please let us     know and we will obtain copies on your behalf).
All medications that are being taken.
Your original health insurance card.

If there are behavioral/developmental issues, please also bring:

• A baby book with developmental milestones.
• Teacher reports from as many years as possible, including early elementary school.
• A written note from current teachers describing their observations.
• Copies (if any) of psychological or educational testing.
  

If unusual episodes are an issue:

Video taping the event is very important.
Also, observe whether the child can be brought out the of the episode by vigorously trying to get his    attention.
